Delaware County is working on its very first Community Health Assessment!

The Delaware County Health Department (DCHD) has begun its first community health assessment, in partnership with community-based organizations and government agencies! The goal of a community health assessment is to identify key health assets, needs, and disparities in Delaware County through comprehensive data collection, research, and analysis. DCHD wants to gain a complete understanding of the public health issues affecting our county and provide accurate data that can be used to develop strategies to improve the health of community members.

How you can help:

Community Health Survey
Residents can help by taking our community health survey. It should take about 10 minutes to complete, and will tell us what health and social problems affect you and your family. The survey is for adults, and is available in several languages.

Community Conversations

Plans are in the works to meet with specific groups, and with the community as a whole. Check back here to learn about opportunities to join in community conversations to discuss health.

Community Partner Survey

This survey is for partners (organizations, groups, government entities) to complete to help us understand what services are provided, to whom, in Delaware County! We are looking for partners to help us prioritize and design/carry out a Community Health Improvement Plan.
