The Delaware County Health Department (DCHD) has proudly concluded its summer outreach series, “Navigating Our Healthcare System for Seniors,” a program designed to help older residents overcome challenges in accessing medical records through online health platforms.
Throughout the summer, DCHD professionals visited nine senior centers across the county, including Chester, Sharon Hill, Media, Folsom, and Upper Darby, providing one-on-one technical support to former Crozer patients. DCHD staff members assisted with medical records requests, health service navigation, blood pressure screenings, and guidance for vital community resources. This initiative was, in part, made possible with the unwavering support of County Council leaders and was inspired by the leadership of City of Chester Health Commissioner Dr. Kristin Motley. DCHD thanks Council Chair Dr. Monica Taylor and Vice Chair Richard Womack for their attendance at some of the sessions, where they heard directly from seniors and reinforced the county’s commitment to improving health access for all.
Seniors who participated in blood pressure screenings also received vital sign information cards. These reminder cards are perfect tools for our seniors' next doctor’s visits, making their latest screening information documented, accurate, and easy to access.

“In a digital world where much of the healthcare system has moved online, many older adults struggle to keep up,” said Delaware County Health Department Director Lora Werner. “DCHD’s goal was to break through the barrier with the assistance from our compassionate and dedicated staff.”
The program's success has drawn significant attention, with calls for DCHD to return and expand into more communities. DCHD is honored and exploring the possibility of future programming in the area of health and digital literacy. DCHD extends heartfelt thanks to the Delaware County Office of Services for the Aging (COSA) and all the participating senior centers, residents, and community leaders who made this program a success for seniors.
