DCHD’s Wellness Center Hosts Navigating Healthcare Event
YEADON, Pa. - The Delaware County Health Department (DCHD) is inviting former Crozer patients and community members to attend the latest installment of its free “Navigating Our Health Care System: Supporting One Another During Changing Times” events on Wednesday, October 15, 2025, from 3:00 p.m. to 6:00 p.m. at the Delaware County Wellness Center, 125 Chester Avenue, Yeadon, PA. The event is free, open to the public, and available on a walk-in basis only. Attendees are encouraged to bring a cell phone and a valid ID if they would like to receive assistance in obtaining their medical records.
With the closing of Crozer and Taylor hospitals this past year, residents were left with little guidance for obtaining their past medical records and transitioning to new providers. In response, DCHD has been hosting “Navigating our Health Care System” events that to date have supported hundreds of Delaware County residents. DCHD plans to continue hosting these events monthly through the end of the 2025.
“Continuity of care is a public-health priority,” said Delaware County Health Department Director Lora Siegmann Werner. “These outreach events are one way we can help make sure that the community doesn’t lose access to care or their medical records.”
