Cold Weather Information: As temperatures continue to drop across Delaware County, residents are encouraged to take necessary precautions during cold weather. Please be mindful of friends, neighbors, older adults, and individuals without shelter or who are in unstable conditions as they are more vulnerable to the cold. A quick check-in can save a life! Spending only a few minutes outside in cold weather can pose a danger to your health, particularly if you are an older adult, have a medical condition, or are otherwise exposed to the elements. Crozer/Prospect Medical Holdings Updates: DCHD continues to provide updates on our website. Please continue to reach out to DCHD’s Wellness Line at (484) 276-2100 for assistance.  Please note, residents can still request their medical records on their own, as well as request a $35 refund if they previously paid for their medical records, on the Morgan Medical Records website.

CHIP: The Delaware County Health Department (DCHD) is pleased to announce the Community Health Improvement Plan (CHIP) 2024-2028, a strategic guide to address key health priorities in the county. Developed through a comprehensive assessment and community collaboration.
