TheDelawareCountyAdministrativeEntitymonitorsallserviceprovidersandSupportsCoordinationOrganizationsservingresidentsofDelawareCountytoensurethattheyfollowPennsylvaniaOfficeofDevelopmentalProgramsguidelines.AdministrativeEntityfunctionsincludeintakeandeligibilitydetermination,providerrelations,serviceauthorization,systemscoordination,qualitymanagement,incidentmanagement,andeligibilityforandcoordinationoffundingsources.TheAdministrativeEntityisresponsibleforimplementingtheAdministrativeOperatingAgreement.

TheDelawareCountySupportsCoordinationOrganizationprovidescasemanagementservicesthatconnectindividualsandfamiliestocommunityresources,bothpaidandunpaid,toprovidethemwiththeopportunityhaveaself-determined,meaningfullifestyle.
TheRoleoftheSupportsCoordinatoristo:
- Linktoschools,theIntermediateUnit,advocacyagencies,serviceprovidersandservices.
- MakesandhandlesreferralsandhelpsindividualsgetservicesidentifiedintheIndividualSupportPlan.
- Isthekeyliaison,communicator,advocate,andproblemsolverfortheindividualtogetservices.
- Monitorservicesandincludinghealthandsafetyconcerns.
- Advocacy/incidentmanagement
- Ensuresallhealthandsafetyconcernsareaddressed.
- Locating/coordinating/monitoringservices.
- IndividualSupportPlandevelopmentandimplementation.
- Needsassessments/behavioral/medical/housing/waiverfunding.
TheSupportsCoordinationOrganizationassistsindividualsandfamilieswithcoordinatingvariousassessments,enlistingtheservicesofwillingandableproviders,andensuringtheeffectivenessofservices.Paidserviceoptionsinclude,butarenotlimited,to:

How to Make a Referral or Interested in Supports Coordination Services
Families must first contact the Delaware County Administrative Entity Intake Specialist to determine if the individual meets eligibility requirements. Contact 610-713-2330 for information.
Once it is determined that you are eligible for supports in the Intellectual Disability system, you will select a Support Coordination Organization, meet with a Supports Coordinator who will develop a support plan, and will then coordinate supports for you and your family.
