Delaware County Health Department to Hold New ‘Navigating Our Health Care System’ Session in Yeadon on Oct. 15

Helping Delaware County seniors and former Crozer patients access care, records, and support

Delaware County, PA — The Delaware County Health Department (DCHD) will host the next session in its “Navigating Our Health Care System for Seniors” series, a monthly initiative providing residents with practical, one-on-one support to access care, retrieve medical records, and connect with health resources across the community.

The free, walk-in event will be held on Wednesday, October 15, 2025, from 3:00 p.m. to 6:00 p.m. at the Delaware County Wellness Center, 125 Chester Avenue, Yeadon, PA.

It’s been nearly two months since Crozer Health officially closed the last of its hospitals and health offices in Delaware County, and many former patients are still struggling to obtain copies of their medical records. County officials note that the challenge has been especially significant for seniors, who often need assistance navigating online systems or requesting documents by phone. Many lack access to computers or email accounts, and delays in response from Prospect have made the process even more difficult.

The Navigating Our Health Care System series was launched early in the summer in response to Prospect Medical Holdings’ abrupt decision to close Crozer Hospital in Upland and Taylor Hospital in Ridley Park, which left thousands of residents —especially seniors— without local care, providers, or access to years of their own medical records.

Held monthly at locations convenient to former patients of Crozer hospitals, the program brings trusted staff and healthcare partners directly to residents, offering one-on-one support to help them reestablish care and reclaim vital records. Since its launch earlier this year, the series has helped more than 270 residents obtain medical records, find new providers, and connect with critical resources. Participants have also received blood pressure screenings, personalized guidance, and referrals to ensure that no one is left behind during this transition.

Delaware County Council commends the Health Department for leading this vital initiative and standing up for residents during one of the most difficult healthcare transitions the county has faced.

“Since the bankruptcy of Prospect Medical Holdings and the abrupt closure of Crozer and Taylor hospitals, our residents have faced serious challenges accessing care,” said County Council Chair Dr. Monica Taylor. “Delaware County stepped up when Prospect walked away. Through our Health Department and partners across the region, we’re helping residents reconnect with care, regain their records, and rebuild trust in our healthcare system.”

Vice Chair Richard Womack praised both the Health Department and the regional hospitals that have helped fill the gap left by Prospect’s withdrawal. “When private equity stripped away vital healthcare services, it was local government and our healthcare partners who stepped up for our residents,” said Womack. “This is what leadership looks like—communities and governments working together to protect people, not profits.”

Delaware County Health Department Director Lora Siegmann Werner noted that the program has been especially impactful for those who attend. “This has been a challenging transition for many of our seniors,” said Werner. “We’ve met residents who felt lost after the hospital closures, and we’re helping them reconnect with care, regain their records, and feel supported again.”

The October 15 event is free, open to the public, and walk-in only. Attendees are encouraged to bring a cell phone and valid ID for assistance in retrieving their medical records.
